Home and Community Based Services

We provide individualized services and supports to persons with intellectual disabilities who are living with their family, in their own home or in other community settings, such as small group homes.

What We Deliver

We deliver comprehensive care to individuals diagnosed with intellectual and developmental disabilities, supporting their health, independence, and quality of life in the Home and Community-based Services program.

Our Vision

Our goal is to go above and beyond in making a meaningful difference in the lives of individuals with intellectual and developmental disabilities in our communities.
